4/19/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ben-Nita. Upon arrival to the home, Ben-Nita appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She indicated to QMHP-C her day was going well. QMHP-C engaged mother in a discussion to determine behaviors since the last session. Ms. Shabazz reports she has trouble with controlling what she says out of her mouth. 2. Behavior: During the second portion of the session, Ben-Nita was a little upset with her mother when discussing her how she talks to people as Ben-Nita was pouting and smacking her lips throughout the session. 1. Intervention: QMHP-C began the session by inquiring about Ben-Nitaâs current thoughts, feelings and emotions. QMHP-C reminded Ben-Nita of ability to demonstrate leadership qualities and encouraged her to do a better job of expressing her needs and feelings without being disrespectful. QMHP-C discussed coping strategies taught to include deep breathing and modeled appropriate responses in order to reduce profanity and aggression toward peers and others in the home. 2. Intervention: QMHP-C encouraged Ben-Nita to work harder to let down walls to openly discuss feelings surrounding sensitive topics as they adversely affect her ability to build healthy relationships and positive self-image. QMHP-C modeled more appropriate ways to communicate with adults and reminded Ben-Nita of potential consequences for being argumentative and disrespectful toward persons of authority. 1. Response: Ben-Nita appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Ben-Nita discussed with QMHP-C on how she has to show leadership skills due to her potential of moving up within the company and how much her managers like her. She discussed she continues to have to tell her grandmother all the time to let her grow up due to her always telling her what she needs to do and what she is not doing as she now feels as though her mother is not going to see her grow up physically on earth but she will have to in heaven which makes her so upset. 2. Response: Ben-Nita discussed with QMHP-C on how she wishes that she could have just gotten one more chance to just do everything with her mother that she wanted to do. She verbalized that although her grandmother is acting as her mother, she will never take her place. She reports that she could run to her grandmother to bail her out of trouble when her mother would get mad at her so it is not the same because she has to do what her grandmother tells her to do which is not fun at this point.
